Medication Therapy Management (MTM)

MTM includes five core elements: medication therapy review,  a personal medication record, a medication-related action plan, intervention or referral, and documentation and follow-up.

MTM is especially effective for patients with multiple chronic conditions, complex medication therapies, high prescription costs, and multiple prescribers.

Follow up during patient transition of care from one setting to another

Hospital discharge is a complex process , representing a time of significant vulnerability for patients. Transitions increase the risk of adverse events due to the potential for miscommunication.

Collaborative Drug Therapy Management AGREEMENT (CDTM)

CDTM between pharmacists and other health care providers, has been shown to be effective in improving clinical and behavioral health indicators, including : 

  • lowering blood pressure, HbA1c, and LDL cholesterol levels
  • improving treatment quality through pharmacist compliance with clinical guidelines
  • increasing patient knowledge and adherence to medication regimens
